Case Manager Registered Nurse- Field - Must live in Louisiana

Position Summary

Captures and coordinates all case management activities with members to evaluate medical needs and enhance overall wellness. Develops strategies to improve member outcomes and opportunities. Trains programs and procedures in compliance with network management and clinical coverage policies.

Minimum Requirements

• Current/Active Clinical Licensure (RN, SW, LCSW, LPC, LMFT)
• Case Management experience
• Strong customer service orientation and problem solving
• Excellent communication and telephonic skills
• Excellent motivational interviewing skills/ability to build rapport and trust telephonically
• Excellence in documentation and respect for compliance/regulatory standards
• Highly organized, self-driven without direct supervision
• Manage time effectively
• Achieve performance metrics
• Technology proficiency
• Ability to travel within 125 miles round trip for 1 day in the field
• Variable work schedule with ability to work 2 days weekly until 9pm local time

Preferred Qualifications

• Bachelor’s and/or Master’s Degree
• CCM Certification
• Previous experience in Managed Care Organization

Pay Range

The typical pay range for this role is $54,095.00 - $116,760.00. This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
