About the Role
HospiceCare in The Berkshires (HCIB) has been caring for generations of neighbors in Berkshire County since 1986. Our team provides support and care for those living with a life-limiting illness, guiding families through emotionally challenging times with information, resources, and expert medical care.
Responsibilities
- Demonstrates understanding of the hospice philosophy/principles and can articulate these effectively.
- Safeguards ongoing management of the plan of care from referral through discharge for hospice patients and families according to regulations and agency standards.
- Conducts initial nursing assessment of patients and families to identify hospice needs.
- Performs comprehensive physical assessments and histories of current and past illnesses.
- Develops and implements a plan of care collaboratively with patients and families, setting goals and interventions aimed at optimal patient outcomes while respecting dignity, autonomy, and comfort.
- Coordinates the delivery of medications, durable medical equipment, nursing supplies, and contracted services with cost-effective strategies.
- Anticipates and prepares for after-hours needs.
- Administers medications and treatments as directed by physicians, observing and documenting responses, and making adjustments as necessary.

Qualifications
Must have a current license to practice as a Registered Nurse in the state of Massachusetts.
Skills
