About the Role
The Refugee & Migrant Long-Term Case Manager will provide holistic and long-term case management to eligible refugee and migrant clients identified as especially vulnerable. The role involves creating self-sufficiency plans at intake and conducting periodic assessments to support clients in achieving long-term self-sufficiency, aligned with the mission of Catholic Charities of Northeast Kansas.
Responsibilities
- Provide case management services for a caseload of refugees and immigrants in the Long-Term Case Management (LTS) program.
- Ensure participants understand their LTS Program rights and responsibilities, and document services and progress in electronic case files.
- Adhere to Catholic Charities of Northeast Kansas’s Refugee & Migrant Services guidelines for service provision and documentation.
- Implement a strengths-based approach to support vulnerable clients in achieving self-sufficiency.
- Receive and manage referrals, participate in case conference meetings, and ensure continuity of care.
- Assess client needs, create Self-Sufficiency Plans (SSP), and meet regularly with clients in alignment with program policies.
- Collaborate with internal and external partners to provide holistic support for clients’ long-term self-sufficiency and integration.
- Respond to crises with appropriate resources and timely interventions to stabilize clients while maintaining safety and ethical boundaries.
- Support clients facing vulnerable situations, including domestic violence, substance abuse, homelessness, and mental health concerns.
- Advocate for clients and facilitate access to services, including transportation, language access, and community partner education.
- Conduct home and field visits as needed, following agency health and safety protocols.
- Maintain professional conduct in accordance with the NASW Code of Ethics and act as a mandated reporter.
- Complete VIRTUS training, background screening, drug testing, and any ongoing training as required.
Requirements
- Bachelor’s Degree in social work or a human service field, or comparable experience.
- Competency in oral, written, and interpersonal communication skills in English; fluency in a second language (e.g., Burmese, Karen, Dari, Swahili) preferred.
- Ability to work effectively in a multi-cultural and multi-lingual environment.
- Strong typing and internet skills, with the ability to learn Microsoft Outlook, Excel, and various database programs.
- Ability to collaborate with outside agencies and advocate on behalf of clients.
- Reliability, punctuality, and professionalism, with the ability to respond appropriately to people in crisis.
- Valid driver’s license, car insurance, a verified safe driving record, and reliable transportation for local travel; must provide a copy of license and insurance.
- Demonstrated passion for the mission of Catholic Charities.
- Willingness to adhere to the agency’s values: love, patience, kindness, faithfulness, self-control, and joy.
- Understanding of Catholic Charities’ role as a visible representative of the Archdiocesan Church, including alignment with Catholic teachings and positions of the Archdiocese of Kansas City in Kansas and the United States Conference of Catholic Bishops.
- Completion of VIRTUS training, background screening, drug testing, and any ongoing training as required.

- Prolonged periods of sitting at a desk, working on a computer, standing, bending, and moving.
- Ability to lift up to 10 pounds.
- Ability to conduct visits in various environments, including walking on rough terrain and climbing stairs.
- Physical ability to effectively communicate with others.
- Physical ability and appropriate licensure for driving.
