Case Manager PRN Flex

About the role

Palestine Regional Medical Center (PRMC) provides comprehensive care, including diagnostic, therapeutic, and rehabilitative services. As a Case Manager, you will play a crucial role in monitoring and managing patient care to promote continuity, optimal outcomes, patient satisfaction, cost efficiency, and compliance.

Responsibilities

• Maintain accurate and up-to-date patient records and ensure compliance with regulatory standards.
• Facilitate communication between patients, families, healthcare providers, and other departments to ensure seamless care coordination.
• Evaluate patient needs and develop care plans that address physical, emotional, and social aspects of recovery.
• Assess patient progress and make adjustments to care plans as necessary to achieve desired outcomes.
• Manage patient transitions between levels of care, ensuring a smooth handoff process.
• Collaborate with interdisciplinary teams to provide holistic care solutions.
• Monitor patient safety and identify potential risks to prevent adverse events.
• Ensure all care activities are documented accurately and timely.
• Provide education and support to patients and their families regarding their health conditions and treatment plans.
• Participate in quality improvement initiatives and contribute to ongoing performance evaluations.

Requirements

• Graduate of an approved school of nursing
• Three years clinical experience
• Critical thinking skills, decisive judgment, and ability to work independently
• Effective communication and problem-solving skills

Qualifications

• Current license to practice as a registered nurse in the State of Texas

Pay
