Case Manager - Prescription Assistance
Summary
As part of the Access to Care team, you work in close communication with our other case managers at Stephens Hospital and across the MaineHealth system to address Social Drivers of Health and provide healthcare navigation and financial resource strain for our patients. Team members utilize a variety of tools, systems, and resources to match patient needs with available resources.
Responsibilities
- Interact with medical providers, pharmacy staff, and staff at pharmaceutical companies to research and resolve recurring and/or urgent medication needs.
- Administer and track the status of Prescription Assistance Program (PAP) applications.
- Advocate on behalf of members and screen program members for eligibility for local/state/federal programs.
- Aid in finding community resources.
Requirements
- Associate or bachelor’s degree in social services, pharmacy, or healthcare field preferred.
- Two years of experience in social/human services, pharmacy, healthcare management, or related field required.
- Community outreach experience and networking required.
- Experience in public speaking required.
