About the role

Catholic Charities of the Archdiocese of Newark (CCAN) is seeking a Case Manager for its Partial Care department. This position supports the severely and persistently mentally ill population, who may also have substance abuse problems. Key responsibilities include providing case management and support, facilitating groups, documenting according to regulatory standards, carrying a caseload, functioning as part of the PC team, interfacing with the PC psychiatrist, and maintaining collateral contacts with family members and providers.
