About the role
The person in this role will coordinate the services for young adults in transition. This may include working with various agencies, providers, faith base organizations, and individual partnerships. They will develop, implement, and manage the individualized plans for young adults ages 16 - 24. They will help to ensure young adults are provided with the tools, support, and resources necessary to become sufficient.
Qualifications
- Education and Experience
- Minimum of a bachelor’s degree in social work or a related human service field
- 2 years’ experience working with transition age youth and/or individuals affected by homelessness, mental health or substance abuse
- Requires general knowledge on “Housing First” philosophy and strategies
- Organizational Skills
- Ability to meet a demanding workload
- Detail-Oriented
- To complete requirements of files and maintain contract compliance
- Communication Skills
- Listening, mediation, and writing skills
- Creative Thinker/Adaptive Personality
- Knowledge or Understanding
- “Strengths-based” case management
- Community resources, social service agencies, and landlords
- Languages
- Fluency in the English language is required
- Spanish speaking ability is an added asset but is not required
- Computer Skills
- Experience with computer
- Knowledge of Microsoft Office
Knowledge or understanding of tenant’s rights and responsibilities as well as “strengths-based” case management a plus.
