Responsibilities
- Conduct In-Home and In-Office assessments using standardized instruments (e.g., PHQ-9, NSI) to evaluate social and emotional needs.
- Develop and implement written case plans, including individual/family counseling and guidance to resolve complex challenges.
- Screens for eligibility and provide advocacy for benefits such as Medicare, Medicaid, SNAP, and public assistance.
- Arrange for essential services including home care, transportation, Meals on Wheels, legal aid, and psychiatric examination.
- Evaluate clients' capacity to manage their affairs; assist in securing financial management or guardianship to prevent exploitation.
- Lead caregiver support groups, conduct community education, and represent JASA at community conferences and meetings.
- Maintain accurate case records and reports utilizing program-specific software; inform supervisors of crisis situations immediately.
Qualifications
- Education: Graduate of an accredited college or university with a BSW, BA/BS, or equivalent degree.
- Technical Skills: Computer literacy, including the ability to learn program-specific software; strong record-keeping and writing skills.
- Communication: Ability to read, write, and speak English; knowledge of Spanish may be helpful.
- Travel: Ability to travel to clients’ homes using mass transit and navigate physical barriers (stairs, etc.) in residences.
