Case Manager NEX

About the role

The Case Manager oversees patient hospitalization from pre-admission to one week post-discharge, ensuring coordinated care and achievement of optimal outcomes.

Responsibilities

• Manage a caseload of patients throughout their hospital stay and post-discharge.
• Coordinate care among healthcare team members to facilitate effective treatment.
• Complete patient education, perform leadership duties, and contribute to quality assurance and performance improvement activities.
• Engage in communication, networking, and systems-based practices to support patient care.
• Provide holistic care addressing physical, psychosocial, educational, and safety needs across all ages.
• Perform additional duties as required.

Qualifications & Skills

• Minimum 3–5 years of relevant experience; experience in case management preferred.
• BSN required; Master’s degree preferred.
• Registered Nurse licensure and current BLS certification required.
• Proficiency in MS Office and EMR software such as Epic required.
• Experience working with all organizational levels necessary.
• Certification such as CCM or CCCTM is preferred.

Schedule

Part-time position (24 hours/week), onsite during days from 8 am to 4:30 pm.
