About the role
The Case Manager oversees patient hospitalization from pre-admission to one week post-discharge, ensuring coordinated care and appropriate outcomes.
Responsibilities
- Manage a caseload of patients, facilitating care coordination among healthcare team members.
- Complete educational activities, perform leadership, quality assurance, performance improvement, communication, and system-based practice duties.
- Provide comprehensive care addressing physical, psychosocial, educational, and safety needs across all ages.
- Perform other duties as required.
Qualifications & Skills
- Experience in case management and working across organizational levels preferred; proficiency in MS Office and EMR systems like Epic required.
- Bachelor's degree in Nursing (BSN) required; Master's preferred.
- Registered Nurse licensure and current BLS certification required.
- 3-5 years of relevant experience; CCM or CCCTM certification preferred.
Schedule
Part-time onsite role (24 hours/week, days 8a-4:30p).
