CASE MANAGER: Mental Health Court St. Lucie

About the role

New Horizons is the largest mental health and addiction recovery provider on the Treasure Coast, serving 15,000 children and adults annually through inpatient crisis services, a 24-hour help line and mobile response team, and outpatient programs across Indian River, Martin, Okeechobee, and St. Lucie counties. The organization also assists 22,000 students in area schools and collaborates with courts, law enforcement, jails, and hospitals to improve individual health and community quality of life.

This position provides screening, linkage, monitoring, and substance abuse outreach services for adult clients under Mental Health Court supervision. The role involves coordinating treatment and services for Mental Health Court participants, assisting in obtaining community resources and benefits, and monitoring, supporting, and reporting progress to the court.

Responsibilities

  • Assess clients being referred to the 19th Judicial Circuit Mental Health Court Project.
  • Complete comprehensive needs assessments for assigned clients to identify needs and services, including outreach for clients requiring substance abuse services.
  • Act as the primary Mental Health Court contact for New Horizons clients participating in Indian River Mental Health Court.
  • Coordinate with other New Horizons staff to help clients access mental health and substance abuse services.
  • Provide assistance to clients for accessing needed services.
  • Participate in committees, meetings, clinical training, staff development, and community relation activities.
  • Prepare written documentation for each service conducted.

Requirements

  • Bachelor’s Degree from an accredited university or college with a major in counseling, social work, psychology, nursing, rehabilitation, special education, health education, or a related human services field.
  • One year of experience.
  • Must pass Level 2 Background through DCF and AHCA Clearinghouse.
