Case Manager - Medical Social Associate (4260)
About the Role
This position provides direct services to individuals through a program funded to offer trauma-informed care for patients/clients who do not often fit well within the standard medical model. You will work closely with the Survivor Recovery Center (SRC) staff, medical providers, primary care teams, and other agencies to improve patient outcomes and care. The role involves conducting outreach and assertive case management to survivors of violent crime who are typically unable to access traditional services. You will collaborate with primary care providers, local health departments, and community-based organizations to provide wrap-around services and build trust for continued care of socially vulnerable populations.
Responsibilities
- Determine client eligibility for services.
- Interview clients and/or families regarding needs related to social determinants of health, which may include home visits, and coordinate recommended social and/or medical services.
- Assist patients in setting and achieving personal goals in person, by phone, or in group settings.
- Through skill-building interventions and other supportive measures, help patients utilize resources, manage symptoms, and develop daily living skills for greater independence, employment, and meaningful activities.
- Initiate or recommend referral services to other agencies to meet patient needs.
- Develop and coordinate service plans, collaborating with agencies to create a network of integrated services.
- Maintain current case records and prepare required reports.
- Collect and maintain up-to-date information about available community resources.
- Arrange and participate in interdisciplinary case conferences as needed.
- Explain university/agency services and programs to the community and deliver educational presentations to community organizations.
- Communicate unmet client needs to responsible parties for appropriate action.
- Participate in interagency collaboration initiatives.
- Act as a liaison between patients and providers to facilitate communication.
- Advocate for clients’ rights.
- Perform other related duties as assigned.
Qualifications
One of the following:
- A Bachelor's degree with a major in social or behavioral science or a related health specialty from an accredited college or university; OR
- A Master's degree in social or behavioral science or a related health specialty from an accredited college or university.
Additionally, one (1) year (12 months) of progressively more responsible full-time experience in social services/agencies or a medical/clinical setting, maintaining acceptable standards of social work practices.
