About the Role

STGi is seeking a qualified Case Manager for the Department of Housing and Urban Development (HUD) and Veterans Affairs (VA) Supportive Housing Program (HUD/VASH). This position provides clinical case management services to a caseload of up to 25 Veterans enrolled in the HUD-VASH program, including assessing household needs, educating households on community resources, developing housing stability plans, scheduling appointments, and providing follow-up to ensure plans progress on schedule. This role requires substantial field/outreach work.

Responsibilities

  • Arrange, coordinate, and provide direct clinical case management and support to a caseload of up to 25 Veterans experiencing homelessness.
  • Assist Veterans in navigating the Housing Authority voucher process and provide support, along with the Housing Locator, to obtain permanent supportive housing.
  • Conduct comprehensive screenings and assessments to collect functional, environmental, financial, employment, housing, educational, and health information to develop a case plan.
  • Provide education on community living and basic life skills (money management, shopping, safety, basic cleanliness, and nutrition).
  • Provide referrals and linkage to VA medical facilities, VA Regional Offices, and community-based agencies for services including health care, benefits, vocational assistance, education, recreation, and other needs.
  • Provide direct mental health and substance abuse counseling using the Harm Reduction Model, within scope of practice, or refer to another provider.
  • Provide crisis management services, including periodic 24-hour coverage.
  • Establish collaborative relationships with service providers inside and outside the VA to ensure continuity of care with wrap-around services.
  • Advocate on behalf of Veterans to assist in delivering needed services.
  • Attend daily/weekly meetings, supervision, and case conferences as assigned.
  • Actively involve Veterans in the development of an interdisciplinary treatment plan.
  • Update Clinical Reminders and other documentation in the CPRS system in compliance with VA requirements.
  • Maintain clear and timely charts in the CPRS system of contact with Veterans and other providers.

Requirements

  • Master's degree preferred from an accredited institution in the social sciences or family therapy.
  • One (1) year of demonstrated case management experience with high needs/high intensity service populations.
  • Minimum of one (1) year experience working with homeless or veteran populations.
  • Knowledge of or experience with DSM diagnoses, assessment of level of functioning, and Evidence-Based Practices such as Motivational Interviewing, Critical Time Intervention, DBT, Harm Reduction, Cognitive Behavioral Therapies, Trauma Informed Care, Housing First, and PTSD.
  • Ability to work collaboratively with others and in a team.
  • Takes initiative and has a solution-focused practice.
  • Good time management skills and ability to balance direct service and paperwork.
  • Excellent verbal and nonverbal communication skills.
  • Current Real ID California driver's license and insurance for regular driving and transporting.

Working Conditions

  • Possible exposure to inclement weather during community visits with Veterans.
  • Work with people experiencing homelessness on streets, in shelters, or other places of habitation may present challenges including exposure to bed bugs or other infestations, unpleasant smells or odors, individuals with poor hygiene or unclean homes due to homelessness, mental health symptoms, or poverty.
  • Employees may encounter profanity, sexually explicit or derogatory language, or verbal or physical expressions of anger and trauma. Negative or derogatory communication patterns can often be negotiated successfully.
  • All direct care staff receive adequate training to develop de-escalation skills, and STGi has established procedural safeguards to mitigate these inherent risks.

  • Pay rate: Starting at $30/hour DOE.
