Responsibilities

  • Coordinate and manage resident care plans in collaboration with the interdisciplinary team.
  • Monitor resident progress toward clinical and discharge goals.
  • Facilitate communication between residents, families, physicians, nursing staff, and ancillary departments.
  • Assist with admissions, care conferences, discharge planning, and transitions of care.
  • Track and follow up on physician orders, appointments, referrals, and authorizations as needed.
  • Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements.
  • Identify and address barriers to successful discharge and continuity of care.
  • Support quality improvement initiatives and resident satisfaction efforts.
  • Participate in interdisciplinary team meetings and care plan reviews.

Qualifications

  • Current and active California LVN license.
  • Minimum one year of nursing experience preferred; skilled nursing or post-acute experience strongly preferred.
  • Previous case management, care coordination, MDS, or discharge planning experience is a plus.
  • Strong organizational, communication, and problem-solving skills.
  • Ability to manage multiple priorities in a fast-paced healthcare environment.
  • Proficiency with electronic medical records and Microsoft Office applications.
  • Commitment to providing exceptional resident-centered care.

Pay

Pay Range: $29.00 - $36.00 per hour DOE
