Responsibilities

• Promote the efficient utilization of skilled services by ensuring that patients are admitted according to medical necessity
• Ensure services are provided to meet the assessed need of each patient in a prompt and efficient manner, and in accordance with patients insurance
• Facilitate that patients are discharged to the most appropriate environment with the necessary supportive services to meet their ongoing care needs
• Ensure facility receives proper authorization of care and level of service

Qualifications

• Licensed LVN
• Previous Case Management experience
• Computer skills
• Communication skills
• Problem-solving skills and attention to detail
• Ability to multi-task, work independently, and manage multiple patient cases concurrently
