Helping Hands provides HOPE and CARE through personal, trauma-informed programs to individuals experiencing homelessness. Our programs are Trauma-Informed, Data-Driven, and Person-Centered. We take the time to learn each individual's story and help connect them to the appropriate local resources and services to meet their unique needs. We do not duplicate available services; instead, we build strong collaborative partnerships with local programs and foster connections between participants and our community partners.
Position Summary
The case manager is responsible for supporting participants in the reentry program. This includes assessing each participant's needs, creating and updating individual reentry plans, ensuring program compliance, and helping participants navigate external resources to achieve their goals. The case manager collaborates with various professionals and maintains confidentiality. The position reports to the Facility Director and works a flexible schedule, including some early morning, evening, and weekend work. The pay is $22.71 per hour, with full-time benefits including medical/dental/vision/PTO/401k. The position is open until filled, with the first review of applications occurring on 7/27/2026.
Key Accountabilities
- Assess each participant’s needs and obtain knowledge of each person’s history through interviewing, records review, and consultation.
- Schedule and conduct monthly meetings with assigned participants to create and update individual reentry plans (IRPs) with new goals, as well as assess the status of prior goals to determine achievement and/or other strategies for completion.
- Help participants achieve and maintain program compliance.
- Aid participants in navigating external resources to support their success, such as scheduling appointments with the DMV, applying for SNAP benefits, referring them to affordable health care and/or housing resources, finding and scheduling with medical providers, job searches, etc.
- Collaborate with other professionals involved in the participant’s success, such as sponsors, doctors, lawyers, DHS workers, counselors, peer support specialists, family members, parole officers, etc.
- Maintain confidentiality of each person involved in our reentry program.
- Crisis Intervention: Consult with participants experiencing a crisis by de-escalating situations using trauma-informed techniques. Identify and facilitate connection with appropriate resources in-the-moment.
- Build a resource network to support participants in a variety of areas, such as: medical, mental health, DUI classes, social groups, financial counseling/coaching, legal support, etc.
- Develop relationships with local property managers, landlords, housing authorities, and developers of affordable housing.
- Maintain current information on housing opportunities, community resources and applicable laws.
- Provide education and resources to participants on how to secure and maintain permanent housing.
- Coordinate the application and payment of rental assistance for program participants.
Education, Skills, Experience And Training Required For The Position
- A High School Diploma or Equivalent
- Minimum of one year of case management in social services experience – an equivalent combination of education and relevant experience may be considered.
- Excellent interpersonal, oral, and written communication skills.
- Excellent documentation skills.
- Basic technology skills, such as: using Microsoft programs (word processing and spreadsheets), using a computer, inputting information into software, using a smart phone, and sending emails.
Preferred Qualifications
- Direct or Lived Experience with homelessness, addiction, and/or low-income populations.
- Understanding of the issues surrounding homelessness, chemical dependency, domestic violence, and sexual assault.
