Case Manager - LifeSet

About the role

The Case Manager provides services to young people transitioning out of care, focusing on skill development, support, and successful transition into adulthood.

Responsibilities

• Provide comprehensive direct services using the LifeSet model
• Conduct individual intensive services, meeting young adults in home and community settings
• Implement interventions and collaborate with external stakeholders
• Engage with youth and conduct ongoing assessments
• Respond to emergencies 24/7 and attend supervision meetings
• Adhere to ethical and legal guidelines, including confidentiality and mandatory reporting
• Develop and revise service plans monthly
• Document all service contacts within required timelines
• Attend and participate in trainings, supervision, and development meetings

Qualifications

• Bachelor's degree in Social Work, Psychology, or related field
• Master's degree in Social Work, Psychology, or related field preferred
• One-year related experience in counseling, case management, or similar field
• Ability to maintain a flexible schedule, manage multiple priorities, and communicate effectively
• Strong organizational skills, attention to detail, and conflict resolution abilities
• Basic computer skills in word processing, spreadsheets, and databases
• Valid Washington State drivers license and liability insurance
• First Aid/CPR certification

Requirements

• Physical demands include standing, walking, bending, lifting, and keyboard use
• Non-physical requirements include education (bachelor's degree in Social Work, Psychology, or related field), experience, and special skills
