About the role
Samaritan Recovery Community is a caring, long-standing nonprofit substance abuse treatment provider serving Nashville and Middle Tennessee. Located in the historic East Nashville area with spectacular views of the downtown skyline, we provide intensive residential treatment, transitional living, intensive outpatient, and outpatient services. We provide treatment services to men and women regardless of their ability to pay.
Responsibilities
- Case Management Responsibilities
- Participate and interact in daily activities of the clients, to develop relationships and trust, as well as help to maintain Samaritan standards during unstructured time periods.
- Supervise phone calls and schedule appointments for clients.
- Work with the treatment team to develop discharge plans for each client, including transfer to Transitional Living (TL), referrals, medical and mental health follow-ups, and present educational material to clients.
- Aid in the development of the service plan for each client, which involves motivating committed treatment, supporting clients in transition, and fostering independence for lasting community involvement.
- Assist the treatment team with making aftercare referrals, including outpatient mental health programs, intensive outpatient programs, and half-way houses.
- Discuss with the Clinical Director weekly and discuss appropriate referrals to Samaritan Recovery Community's TL program.
- Aftercare role including developing essential life skills and establishing long-term goals.
- Coordinate with clients who have legal concerns, including those on state probation.
- Send weekly progress reports to Forensic Social Workers.
- Send admission/discharge faxes to concerned parties within the legal system.
- Occasional transport of clients to community appointments in the area.
- Connect with housing administrations, childcare facilities, advocacy groups.
- Be included in the IR Lecture/Seminar rotation.
- Utilize resources in the community to aid clients with obtaining driver’s license, social security card, birth certificate, food stamps, TNCare, grants for sober living/housing, etc.
- Document activities in approved systems - progress of each client’s participation is a key element to this position.
- Other related duties as assigned.
Requirements
- High School Diploma, some college preferred.
- Driver’s License with “F” endorsement.
- Understanding of recovery process and treatment.
- Able to work independently and with a team.
Physical Demands / Work Environment
- Must be able to sit/stand for 8 hours.
- Must be able to walk/stand for up 50% of shift.
- Walking, bending, light lifting up to 50 lbs.
