Job Summary

The Behavioral Health Community Case Manager provides direct and collateral support to individuals who are addicted to opiates or in recovery from this addiction to facilitate consumer access/participation in all community supports needed to begin or sustain recovery. The Community Case Manager also supports consumers’ success in recovery by providing crisis support, and assisting with tasks related to achieving Stages of Change objectives.

Job Responsibilities

  • Provide in person community based crisis outreach/engagement to the local emergency room, law enforcement, crisis hotline and other formal/informal providers working with or responding to individuals with opiate addiction.
  • Foster close working relationships with individuals in the provider, law enforcement, faith, business, government, and recovery community in order to become recognized, accepted and utilized as a resource for individuals with opiate addiction.
  • Provide integrated case management support for individuals with opiate addiction from point of first contact through each step in a Stages of Change treatment model including: detoxification; hospitalization; community based aftercare; support groups; and all community supports that support an individual during recovery.
  • Case management support will include collateral contacts related to: access to detox services and treatment; housing; insurance/benefits; medical care; transportation; facilitating communication between providers; setting up and facilitating attendance at appointments; education; in-person and telephonic supportive counseling; routine non-clinical correspondence; and miscellaneous contact with family members and community providers.
  • Meet consumers in the emergency department to engage with individuals and make available community outreach services to facilitate the successful access to and engagement with services recommended during emergency department visits.
  • Serve as a resource to local law enforcement so that officers can provide referrals to the community outreach worker to engage with individuals and assist them in connecting to community services.
  • Attend appointments with the consumer whenever indicated to provide support and linkages to other providers also involved in the consumer’s recovery.
  • Cookier transportation for patients traveling to and from outpatient services including detoxification, agency appointments, medical and behavioral health appointments, and support group meetings.
  • Complete all assigned tasks related to quality assurance data collection and data entry as required by the department, grant sources, state agencies, and insurance bodies.
  • Assist consumers in completing benefits applications for state assistance, financial aid and other miscellaneous entitlement programs.
  • Aid consumers with completing application paperwork for employment, courts, state agencies and local non-profit programs.
  • Complete all assigned documentation responsibilities for collateral contacts and psycho-education groups that meets established Joint Commission, state regulatory and corporate compliance criteria.
  • Attend and participate in community meetings related to opiate addiction and individual agency meetings when invited.
  • Attend hospital based Multidisciplinary treatment team meetings, staff development meetings, and mandatory department/hospital meetings.
  • Follow all established department and hospital policies and procedures.
  • Complete continuing education requirements as required by the unit and hospital.
  • Support to medical and clinical staff in all Department of Psychiatry programs as assigned.
  • Perform all other duties as assigned.

Qualifications

  • Education/Certification: Bachelor’s degree in human services or related field.
  • Experience: Minimum of one year experience in a substance abuse treatment setting.
  • Skills: Proficient in MS Word, Excel, and Outlook.
