Case Manager II

POSITION SUMMARY

Volunteers of America Mid-States (VOA) is a non-profit organization providing various services including housing, care for individuals with developmental disabilities, and free HIV testing and education. The Case Manager II position assists clients in accessing medical and mental health services, emergency and long-term housing, substance abuse services, and social services.

QUALIFICATIONS

• Master of Social Work or related field with less than five years of work experience, or a Bachelor of Social Work or related undergraduate degree with more than five years of work experience, or nine years of experience in the field with no degree.
• Demonstrated ability to provide a reasonable combination of knowledge, abilities, and skills including family needs assessment, knowledge and ability to access local resources, advocacy, counseling, and working cooperatively with local service providers.
• A commitment to the principles of the project, excellent verbal and written communication skills, high degree of organization, and creative problem-solving skills.
• A personal automobile, valid driver’s license, liability insurance, and willingness to travel within the full-service area.

RESPONSIBILITIES

• Provides outreach to community partners and represents VOA values in interactions with community partners to increase awareness and recruit landlords.
• Performs assessment and develops/monitors case plans, conducts necessary follow-up activities, establishes linkages with appropriate agencies, provides referrals, coordinates with community partners and other service providers, decides resource allocation, educates participants on issues, assists in obtaining public benefits, assists in obtaining documents, provides/refers for supportive services, completes required documentation, submits documentation to HMIS and Electronic Health Record system, performs PQI duties, facilitates CTI model, and demonstrates good clinical judgment and cultural appropriateness.
• Advocates for participants, identifies systemic barriers, and communicates with leadership to find solutions.
• Complies with program and organizational policies and procedures.
• Inspects and submits rental payments via Doclink.
