Case Manager II
Since 1984, Larkin Street Youth Services has helped more than 75,000 young adults in San Francisco with outreach, shelter, housing, education, employment, healthcare, and other services. Today, Larkin Street is the largest provider of housing and services to homeless youth in the Bay Area, serving 1,500 youth annually through more than 500 emergency shelter, transitional, supportive, and subsidy-based housing beds across San Francisco. About the role
Under the direction of the Program Manager, you will provide comprehensive, strength-based case management services to homeless and/or marginally housed transitional aged youth (ages 18-24). This includes conducting bio-psychosocial evaluations, developing individualized service plans, and leveraging community resources. The role requires a thorough understanding of the stages of change and development, and comfort providing services in various settings such as emergency shelters, congregate living housing, scattered site housing, and single room occupancy (SRO) placements throughout San Francisco. This is a full-time, onsite position eligible for full benefits.
Responsibilities
- Conduct intake, assessment, direct service linkage, referral, case planning, coordination, advocacy, follow-up, and aftercare.
- Develop and maintain strong relationships with clients.
- Provide crisis prevention, intervention, safety planning, stress management, and life skills development.
- Assist young people in applying for and obtaining benefits and housing.
- Connect clients with outside agencies and supportive services promoting health and wellness.
- Collaborate with community behavioral health providers and internal service teams for coordinated care.
- Maintain accurate observation and progress records; complete electronic data entry into agency and city databases.
- Adhere to HIPAA and reporting requirements; maintain compliance documentation.
- Collaborate with staff, property managers, and landlords.
- Facilitate transportation and accompany clients to community appointments.
- Complete safety and wellness checks in residential units.
- Teach life skills (e.g., cooking, cleaning, budgeting).
- Participate in organizational meetings (e.g., all-staff, CQI, Client Practice).
- Provide services during evenings and weekends as program needs dictate.
- Support program services within the milieu, including crisis intervention and safety maintenance.
- Perform other duties contributing to the care and welfare of clients.
Locations
This is a pooled job posting with potential placement at any of the following programs:
- Residential Programs: Located throughout San Francisco, including the Tenderloin neighborhood. Hours: 9AM–5PM or 10AM–6PM. Working days: Tuesday–Saturday or Sunday–Thursday (includes weekends).
- Rapid Re-Housing Programs: Based in San Francisco with home visits and case management sessions across the Bay Area (within 50 miles, accessible by public transportation). Hours: 9AM–5PM or 10AM–6PM. Working days: Tuesday–Saturday or Sunday–Thursday (includes weekends).
- Drop-In Center: The Engagement and Community Center (ECC), 134 Golden Gate Ave, San Francisco. Hours: 9AM–5PM or 8AM–4PM. Working days: Monday–Friday.
Requirements
- Passion for Larkin Street’s mission to end youth homelessness.
- Alignment with core values and strategies: client-centered, diversity/equity/inclusion, trauma-informed care, restorative practices, harm reduction, non-violent crisis intervention, and motivational interviewing.
- Social services case management experience (experience in drop-in services, shelter, transitional, or permanent supportive housing is a bonus).
- Commitment to serving transitional aged youth (TAY, ages 18–25) of diverse backgrounds.
- Self-starter, team player, compassionate, dependable, and patient.
- 100% onsite/in-person role.
- Willingness to work holidays occasionally.
- Valid driver’s license.
