Salary & Benefits
- Level I (H21A): $40,000.87 plus full State employee benefits
- Level II (H21B): $43,000.94 plus full State employee benefits
Travel
Travel is extensive. Must possess a valid driver’s license and maintain required car insurance.
Minimum Qualifications
- Case Manager – Level I: Bachelor’s degree in a human services field* and one (1) year of professional experience working directly with individuals with intellectual and/or developmental disabilities (or related fields such as social work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, nursing). OR a valid permanent Oklahoma nursing license and one year of relevant experience. A combination of education and experience is acceptable. *A human services degree excludes degrees in physical, natural, biological sciences, or mathematics.
- Case Manager – Level II: Bachelor’s degree in a human services field* and two (2) years of professional experience working directly with individuals with intellectual and/or developmental disabilities (or related fields). OR a valid permanent Oklahoma nursing license and two years of relevant experience. A combination of education and experience is acceptable. *Same definition as above.
Job Responsibilities
Under the direction of a case management supervisor, the position provides services to individuals with intellectual and/or developmental disabilities living in their own homes or community living alternatives; obtains in‑depth social histories; develops and assures community and/or agency resources to meet the consumer’s needs; serves as QIDP and provides monthly monitoring of the program. This position must comply with Federal Home and Community Based Waiver regulations, state, and policy guidelines.
