Case Manager I

Position Responsibilities

• Performs a range of social work duties, including gathering and analyzing data and providing services that match client needs.
• Accompanies clients to medical, public benefits, and other appointments.
• Affirms client needs and coordinates referrals for employment, English language, legal, and other services.
• Develops and implements skills in providing appropriate services with increasing responsibility for the appraisal of the effectiveness of own work.
• Performs record-keeping functions and other administrative duties as required by the Agency.
• Participates in agency staff development programs and professional group seminars or conferences to increase professional knowledge and to report on own work.
• Manages emergency situations.
• Participates in representing JCFS and its programs in a positive manner to both internal and external contacts.
• Performs other duties as assigned.

Requirements

• Bachelor's degree in social work or a related human services field preferred; or equivalent combination of education and relevant work experience will be considered.
• Valid Illinois driver’s license, a safe driving record, and regular access to a reliable, insured vehicle.
• Ability to work a flexible schedule including weekend and evening hours when necessary.
• Proficiency with Outlook, Word, Excel, Teams and other basic computer programs.
• PREFERRED: Fluent in English and at least one additional language representative of the clients served.

Compensation

For this position is starting at $48,000 per year, depending on your experience.
