About the role
The Case Manager, located in South Carolina, works in a hybrid schedule (Monday to Friday, 8:30am-5:00pm) with occasional travel to cover different states. They provide specialized services related to home studies and post-release case management under the guidance of the Regional Manager.
Responsibilities
- Affirms and recommends reunification of children with potential sponsors through interviews and background checks, documenting findings in reports within set timelines.
- Conducts home visits to meet with children and their sponsors post-release, coordinates referrals, and plans services.
- Works with children and sponsors to ensure progress on individual service plans, assessing changes impacting safety.
- Maintains relationships with community services, serves as a point person for stakeholders, and participates in case management meetings.
- Maintains accurate records, ensures confidentiality, and submits required reports and documentation on time.
- Provides crisis intervention and involves Child Protective Services when necessary.
- Travels for home visits as needed.
Requirements
- Bachelor of Social Work (BSW) degree or equivalent degree in education, psychology, sociology, or other behavioral science.
- Minimum one (1) year experience in family reunification, home assessment, report writing, family preservation, or general child welfare.
- Valid and active state driver's license or the ability to obtain one within 15 days of employment.
Qualifications
- Fluency in a second language, especially those relevant to the client base.
- Excellent communication skills, including the ability to speak effectively before groups.
- Knowledge of local community social services, family preservation, kinship care, and general child welfare.
- Experience working with immigrant populations.
- Strong interpersonal skills and the ability to collaborate with diverse work teams and stakeholders.
- Proficiency in Microsoft Office, Internet Explorer, Outlook/Exchange, and Windows operating systems.
- Ability to solve practical problems, interpret instructions, and maintain confidentiality.
