Case Manager - HSPRS

About the Role
Our Home Study and Post Release Services (HS/PRS) program supports unaccompanied children (UC) aged 0–17½, referred through shelters or foster care providers. Case Managers conduct home studies to assess safety and suitability before unification, then provide ongoing post-release services to ensure the child and sponsor thrive. This includes connecting them to medical care, education, mental health services, and other community resources while monitoring their well-being.
Responsibilities
- Ensure safety for all youth on caseload
- Manage a high-turnover caseload of approximately 40 youth (HS and PRS cases)
- Comply with agency and regulatory policies and procedures
- Coordinate initial intake for each youth
- Conduct home studies to assess safety and suitability of sponsors
- Contact each child and sponsor within 24 hours of case assignment
- Conduct safety screenings and mental health assessments
- Coordinate, schedule, and confirm necessary services for youth
- Communicate regularly with youth and sponsors to address needs/concerns
- Maintain current assessments, reports, and case notes in hardcopy and electronic files
- Ensure each youth is enrolled and attending school
- Respond to client emergencies within or outside work hours
- Travel throughout the region (up to 3 hours) and potentially outside the assigned region for home visits and in-person case contacts
Requirements
- Bachelor’s degree in a relevant behavioral/social science field
- Experience conducting home studies according to state requirements
- Strong professional writing skills, particularly for assessment reports
- Knowledge of local community social services
- Experience working with immigrant populations
- 1–3 years of progressive case management experience (preferred)
- Child welfare experience (preferred)
- Ability to work in a fast-paced environment
- Proficiency in Microsoft Office
- Ability to work with culturally diverse populations, non-English speakers, and LGBTQ youth
- Bilingual (English/Spanish) preferred; English writing proficiency required
- Valid driver’s license and reliable personal vehicle for home visits (up to 3 hours away)
- Ability to travel extensively, including by air
Physical Requirements
- Prolonged computer work and desk sitting
- Ability to lift up to 30 pounds
- Frequent driving and/or public transportation for client visits
Schedule
- Exempt position, Monday–Friday, 9am–5pm (business hours)
- Flexibility required to meet program needs, including afternoons, evenings, and weekends
Pay
$64,500 annually
