Case Manager, Housing ($1000 HIRING INCENTIVE)

Responsibilities

• Provide Person Centered Case Management
• Client Skills Groups and Community Supports
• Develop and Maintain a Working Knowledge of Available Community Resources
• Act as a Role Model for Clients
• Maintain Electronic Health Records
• Safeguard Personally Identifiable Health Information (PHI)
• Flexible Schedule to Meet Case Load Needs

Requirements

• Current and Valid Driver's License, Clean Driving Record
• Associate’s Degree and 2+ Years of Experience or Lived Experience in Human Services OR Bachelor’s Degree in Human Services or Related Field
• 1+ Years of Professional Experience in Human Services
• Intermediate Computer Skills
• Relate to People from Diverse Backgrounds with Multiple Individual and Systemic Barriers to Success
• Excellent Verbal and Written Communication Skills
• The Capability of Working Alone with Very Little Supervision

Pay and Benefits
