We're looking for a compassionate, dedicated Case Manager to join our team and support individuals on their journey toward recovery, independence, and community connection. In this role, you'll work closely with participants, their support systems, and community partners to ensure each person receives the services and resources they need to thrive. You'll be an advocate, problem-solver, mentor, and trusted support person, helping individuals navigate challenges while celebrating their successes along the way. What You'll Do As a Case Manager, you will: Partner with participants to identify strengths, needs, and goals. Develop and support individualized recovery and service plans. Serve as an advocate to help participants access essential services and resources. Work as part of a multidisciplinary team that provides holistic, person-centered care. Assist with recovery plan development and ongoing reviews alongside participants, natural supports, and approved stakeholders. Provide community-based services focused on education, skill-building, community engagement, and goal achievement. Build and maintain relationships with community agencies and service providers. Identify service gaps and recommend strategies to improve supports and outcomes. Complete assessments, documentation, and reports in accordance with agency, licensing, accreditation, and funding requirements. Utilize collaborative documentation practices during treatment contacts. Participate in a rotating on-call schedule, including crisis intervention support. Provide at least 75% of treatment contacts within the community setting. Deliver culturally responsive and inclusive services that honor and respect each person's unique identity, experiences, and needs. Maintain confidentiality and comply with HIPAA regulations. Perform other related duties as assigned. Required Qualifications Bachelor's degree in Human Services or a related field. Minimum of two (2) years of relevant experience. Intermediate computer skills. Valid driver's license. Ability to pass Federal and State criminal background checks. Preferred Qualifications Previous Case Management experience. Travel throughout the community. Navigate stairs and uneven surfaces. Bend, stoop, kneel, crouch, and reach as needed. Push, pull, and move materials. Lift and carry up to 50 pounds occasionally, with frequent lifting of lighter items. Use hands and fingers for computer and documentation tasks. Communicate effectively in person, by phone, and electronically. Working Environment This role is primarily community-based and may involve exposure to a variety of environmental conditions, including weather changes, noise, dust, and other conditions encountered while providing services in the community.
