About the role

The Case Manager provides progressive housing stability case management and advocacy for participants in the Homelessness Prevention Program. This role involves accepting referrals of individuals and families at imminent risk of homelessness in Pierce County, implementing a rapid intake process, and developing a housing stability and rental assistance plan. The Case Manager will conduct housing searches, engage with landlords, make referrals to needed services, and work with families to modify plans as service needs evolve, ultimately achieving permanent unsubsidized housing.

Responsibilities

  • Assist referred participants to verify and document program eligibility and complete application materials.
  • Demonstrate cultural and socioeconomic sensitivity in delivering client services.
  • Conduct comprehensive in-depth assessments using intake, assessment, and housing barriers tools.
  • Utilize motivational interviewing to develop a strengths-based plan of action for participants.
  • Provide coaching and conflict resolution services to resolve housing-related conflicts (e.g., between clients and landlords, property managers, or neighbors).
  • Assist participants with referrals to employment, pre-employment, life skills programs, and other customized services.
  • Help participants find and obtain safe and stable housing, including completing rental applications.
  • Submit financial assistance requests for client rent subsidies and other approved expenses.
  • Reassess participant eligibility at 90-day (or shorter) intervals to determine ongoing need for rental subsidy.
  • Maintain up-to-date and accurate client files, including proficiency in Homelessness Management Information System (HMIS).
  • Cooperate on data collection and analysis related to participants.
  • Maintain strict confidentiality of participant information in compliance with HIPAA standards.
  • Participate in ongoing training, program development, and relevant community activities.
  • Perform related duties as assigned.

Requirements

  • Associate's degree in a related field or equivalent work experience.
  • At least 2 years’ experience in a related field.
  • Well-developed written and verbal communication skills.
  • Strong organizational skills.
  • Proficient in Microsoft Word and Excel; willingness to learn new databases.
  • Experience working with participants from diverse backgrounds and perspectives.
  • Experience in community and human services, including serving homeless and low-income communities.
  • Experience providing community presentations.
  • Ability to demonstrate cultural and socioeconomic awareness in client services.
  • Ability to make independent decisions when circumstances warrant.
  • Ability to enter and retrieve data using computer systems and office equipment.
  • Ability to work with ill, disabled, emotionally upset, or hostile individuals.
  • Washington State Driver’s License and proof of current auto insurance required.
