Case Manager - Full-time
About the role
UF Health Rehabilitation Hospital North, affiliated with Select Medical, is an inpatient rehabilitation hospital committed to providing exceptional and compassionate care for individuals with brain injuries, spinal cord injuries, neurological disorders, orthopedic issues, amputation, and multiple traumas. The Case Manager coordinates health care decisions using a systematic approach to assure treatment plans that improve quality and outcomes, coordinate care across the continuum, promote cost-effective care within the allotted time frame, assure payment of hospital-based services meeting patient-related utilization management criteria, and implement safe and appropriate discharge plans. The Case Manager also assesses the psychosocial needs of the patient and provides intervention as part of the discharge planning process.

Requirements
- Current licensure per state guidelines in a clinical or related discipline OR a Bachelor's or Master's degree in a health or human services discipline
Qualifications
- Previous experience in Case Management and Discharge Planning preferred
- CCM Certification preferred
