Case Manager - Full-Time

About the Role

This position involves providing targeted case management services to support individuals recovering from mental illness. The primary goal is to stabilize clients, prevent hospitalization, and promote recovery.

Responsibilities

• Coordinate integrated services and supports tailored to each client's recovery goals.
• Develop and implement Individual Community or Family Support Plans, fostering independence and well-being.
• Collaborate and educate clients' families and significant others to aid recovery efforts.
• Ensure services are delivered with dignity, respect, and cultural sensitivity, maintaining continuous monitoring and communication of client needs.

Requirements

• Bachelor’s degree in social work, psychology, or related human services field with at least 2000 hours of supervised mental health experience, or relevant community-based experience if without degree.
• Strong assessment skills and knowledge of local community resources.
• Ability to work collaboratively, respecting client diversity and promoting recovery.
• Language skills in Spanish, Hmong, or Somali are advantageous, with potential bonuses.

Schedule

Provides services primarily during daytime hours.
