About the role

The Case Manager is responsible for overseeing patient hospitalization from pre‑admission to one week post‑discharge, ensuring coordinated care to achieve optimal outcomes. The role evaluates and assesses patients' needs for safe discharge, facilitates care planning, and connects families with community resources. It involves developing educational materials, communicating effectively with patients, families, and healthcare teams, and demonstrating leadership to resolve conflicts. Candidates must have healthcare experience, excellent communication skills, and proficiency in MS Office and EMR software.

Responsibilities

• Manage patient care throughout hospitalization and post‑discharge.
• Coordinate medical services and community resources.
• Develop and implement patient and staff educational materials.
• Promote effective communication and conflict resolution within teams.
• Assess and meet the physical, psychosocial, educational, and safety needs of patients.

Qualifications

• Licensed registered nurse in Ohio with a BSN (master's preferred).
• 3–5 years of relevant healthcare experience; case management experience preferred.
• Certification in case management is a plus; BLS certification required.
• Strong interpersonal, organizational, and communication skills.
• Ability to interpret data and navigate hospital environments.
• Proficiency in Microsoft Office and EMR software.
