Case Manager - Float

About the role
This is a full-time, day shift position working Monday through Friday. We're offering a generous sign-on bonus of up to $10,000 to welcome you to our team; details will be discussed during the interview process.
This position requires direct patient care.
Responsibilities
- Discuss assessment findings and patient status with physicians and/or the treatment team to maximize outcomes.
- Assist in discharge planning as required.
- Communicate effectively and efficiently internally and externally.
- Coordinate discharge and aftercare services with guidance from the clinical team.
- Support the team and demonstrate follow-through with requests, sharing critical information in a timely manner.
- Initiate a complete assessment to gather and verify relevant baseline data, identify patient understanding and expectations, and address potential barriers to post-inpatient care.
- Proactively work with patients, caregivers, and the interdisciplinary team to develop a realistic and timely plan for post-inpatient care.
- Identify and coordinate appropriate post-discharge resources for patients requiring homecare, outpatient services, or placement in an alternate level of care.
- Perform ongoing evaluation for appropriateness of the facility’s level of care by applying knowledge of level-of-care criteria in collaboration with the patient’s insurance payer.
- Comply with all applicable state and federal regulations, as well as credentialing bodies such as the Joint Commission and CARF, regarding the case management process.
Requirements
- Bachelor’s degree in a related field of study (experience cannot substitute for the degree).
- Current Massachusetts license as an RN, PT, OT, SLP, RT, or LCSW/LICSW.
- 2–3 years of case management, utilization review, or discharge planning experience (preferred).
- Case management experience in a Long-Term Acute Care (LTAC) setting.

Pay
Starting pay range: $69,410.02 per year (Grade 6). Offers will reflect skills, relevant experience, education, certifications, and other factors. Total compensation includes competitive base pay, comprehensive benefits, career advancement opportunities, differentials, premiums, bonuses, and recognition programs.
Schedule
Full-time, day shift (Monday through Friday). Scheduled weekly hours: 40.
