Case Manager/Family Support Worker, Greenfield
About the role
The Case Manager/Family Support Worker will provide community-based direct care services to children, adults, and families, focusing on alleviating mental health issues and strengthening family structures.
Responsibilities
- Provide skills training and assistance in areas of daily living activities including basic living, interactive and parent skills.
- Aid in developing communication and conflict resolution skills and act as a role model for interpersonal relationships.
- Support parents in meeting children's daily needs; observe and document child growth and development.
- Assist with family advocacy needs; facilitate "mediation" as appropriate.
- Offer counseling and intensive support to consumers during psychiatric crises or periods of transition.
- Help consumers identify appropriate mental health and medical services and access services of choice.
- Support the development and maintenance of a "natural support network" of family and friends.
- Provide transportation for consumers as appropriate, following program guidelines.
- Maintain appropriate contacts with family members, others in the "natural helping network," outpatient therapists, and others as necessary to support consumers.
Requirements
- Bachelor's Degree in a mental health field.
- Experience working with children and families.
- Valid Driver's license and reliable transportation.
- Bilingual candidates are strongly encouraged to apply.

Pay
$38,000 - $44,000/year
Schedule
