Qualifications
- Education: A bachelor's degree in a generally recognized clinical discipline.
- Experience:
- Case Manager I: Two years' experience providing case management and/or drug and alcohol counseling.
- Case Manager II: Three years' experience providing case management and/or drug and alcohol counseling.
- Case Manager III: Three years' experience providing case management and/or drug and alcohol counseling including at least one year supervisory or administrative experience.
Responsibilities
- Treat all clients and staff with dignity and respect.
- Attend all team/staff meetings as required by Supervisor.
- Role model pro-social behavior.
- May be responsible for conducting and documenting intake interviews, as needed.
- Be responsible for a caseload of clients, as identified by program requirements and in accordance with state regulations.
- Complete progress notes for all client-related activities within 24 hours of service delivery.
- Cookordinate with agency and community resources to ensure thorough treatment and discharge plans.
- Establish and maintain positive relationships with service providers and referral sources.
- Assist clients with connecting to resources needed to reach personal goals during the course of their treatment and in advance of discharge.
- Arrange access public assistance, social security, food stamps and medical assistance appointments to ensure timely and continued receipt of benefits.
- Arrange medical appointments, psychiatric evaluations, and medication, when appropriate.
- Refer clients to appropriate vocational and educational programming.
- Assist clients in developing a thorough discharge plan addressing their most significant needs.
- Maintain contact with clients after discharge to ensure coordination with other case management/social services/substance use disorder treatment or services.
- Ensure compliance with all federal, state, local and funding source guidelines.
- Maintain valid driver's license.
Preferred Experience
- Licensed by the Department of State or be a Certified Allied Addiction Practitioner or Certified Associate Addiction Counselor, Certified Alcohol and Drug Counselor, or Certified Advanced Alcohol and Drug Counselor at the time of hire or being working toward licensure or certification with a completion date of no more than three years from the date of hire.
