The Case Manager – Emergency Shelter is responsible for finding safe, permanent housing for individuals and families who need short-term emergency shelter. The Case Manager works with participants to identify their needs and coordinate services such as healthcare, job search, and skill-building to prepare for living independently. Case management services target the individual or the whole family, addressing barriers to housing stability.

Responsibilities

  • Provide case management in an Emergency Shelter setting.
  • Develop and implement short-term service plans for participants to address their immediate housing needs.
  • Manage caseloads using a trauma-informed care perspective, harm reduction principles, and a participant empowerment approach.
  • Assess and address barriers and challenges to housing, including financial, legal, or housing-related concerns.
  • Accompany and transport residents to community-based appointments as appropriate.
  • Facilitate and provide opportunities for open accessibility of services, including drop-in office hours and evening appointments.
  • Participate in an integrated care model, working with participants’ primary healthcare providers, state agencies, and other collateral contacts within participants’ care.
  • Provide care coordination and case management services based on clients’ individual or family needs.
  • Provide crisis intervention, conflict resolution, and de-escalation services and support.
  • Conduct risk assessments for participants whose condition, behavior, or circumstances represent a risk to themselves or others.
  • Maintain current and professional written case records for all assigned participants, including records in the electronic health record and the Homeless Management Information System (HMIS).
  • Complete measurable and objective goals and reports to ensure appropriate contract reporting, data collection, and record-keeping.
  • Attend internal and external meetings and training as required or needed.
  • Provide on-site support and collaboration with other staff to ensure program efficacy and effectiveness.
  • Participate in and assist with all participant events as required or needed.
  • Involvement in additional program development activities as assigned.

Requirements

  • Bachelor’s degree from a four-year college or university.
  • At least 3 to 5 years of professional experience in providing case management to adults and/or families.
  • Demonstrated experience assisting individuals or families with financial, legal, and other challenges that create barriers to securing and maintaining housing, such as mental health, substance misuse, and low income.
  • Knowledge of housing and housing-related services and resources, including tenant/landlord rights and responsibilities.
  • Ability to work independently and as part of a team.
  • Ability to demonstrate flexibility and creativity in problem-solving.
  • Strong verbal and written communication skills.
  • Strong time-management and organizational skills.
  • Proficiency with Microsoft Office products, including Outlook, Word, and Excel.
