Overview
Captures and coordinates patient care across a multidisciplinary care continuum within a clinical service unit structure. Ensures optimal resource utilization, service delivery, and compliance with clinical cost education initiatives. Facilitates best possible patient outcomes by advocating and managing resources to achieve patient-centered solutions.
Responsibilities
- Maintains appropriate documentation
- Assesses and evaluates patients' ability to provide self-care; develops discharge plans and goals of care with patients and involved caregivers
- Affords referrals to related agencies while considering patient freedom of choice
- Provides information to social service/discharge planners to facilitate discharge planning processes
- Participates in multidisciplinary rounding and care conferences with other departments
- Serves as a resource person to physicians and hospital staff in the appropriate use of health care resources
- Serves as a liaison with nursing and medical staff and community agencies to facilitate problem-solving and coordinate services
- Assesses and develops an early admission plan and revises as necessary based on patient needs
- Accepts on-call shifts as scheduled and responds as needed
- Takes responsibility for upgrading professional skills and knowledge
- Prepares and completes reports; participates in special projects
- Demonstrates clinical competency by completing BLS CPR and ACMA Compass training modules assigned by the Director of Case Management
Qualifications
- Bachelor’s degree in human services, psychology, sociology, social work, or related healthcare field is required
- Licensure, certification, or registry as applicable
- Experience in healthcare settings
- Good communication and interpersonal relationship skills
- Ability to organize work with minimal supervision
- Ability to focus on minute details
- Access, Word, and Excel computer skills required
