Case Manager/Discharge Planner- PRN
Position Overview
Helen M Simpson Rehabilitation Hospital is a part of UPMC and Select Medical Partnership. This position is a Case Manager located inside UPMC West Shore with a per diem schedule looking for availability during weekday daytime hours. The compensation is $40.00 per hour.
About the Role
The Inpatient Rehabilitation Hospital is dedicated to providing exceptional and compassionate care to address medical, physical, emotional, and vocational challenges for patients with various conditions including brain injuries, spinal cord injuries, neurological disorders, orthopedic issues, amputations, and multiple traumas.
Responsibilities
- Clinical Interventions/Discharge Planning: Coordinate health care decisions and develop treatment plans to improve quality and outcomes.
- Care Planning Management: Manage care plans across the healthcare continuum.
- Fiscal Management: Ensure cost-effective care within the designated timeframe.
- Payer/Referral Management: Implement safe and appropriate discharge plans and manage insurance and referral processes.
- Assess Psychosocial Needs: Evaluate the psychosocial needs of patients and provide necessary interventions as part of the discharge planning process.
Qualifications
- Current licensure per state guidelines in clinical or related discipline OR a Bachelor's or Master's degree in a health or human services discipline.
- Previous experience in Case Management and Discharge Planning is preferred.
- CCM Certification is preferred.
