Case Manager - Developmental Services

About the role

Join our dynamic and energetic team of professionals committed to providing high quality Case Management services to adults (21+) with developmental and/or intellectual disabilities and/or Traumatic Brain Injuries (TBI).

Responsibilities

• Organize, facilitate, and document service planning for individuals with Developmental Disabilities.
• Work collaboratively with other community resources to ensure the quality of service provision.

Requirements

• Associate’s Degree required. Bachelor’s Degree preferred.
• Two-Three years of relevant field experience preferred.
• Excellent oral and written communication skills.
• Ability to utilize provided technology to perform requirements of the job.

Qualifications

• Valid driver’s license and proof of vehicle liability coverage.
• Excellent multitasking abilities.

Skills

• Ability to communicate effectively both orally and in writing.
