About the role

The Case Manager Family Consultant is responsible for maintaining comprehensive records of individuals served, including documentation of services provided and progress made. They also participate in various team meetings and rotations, serve as an advocate, and ensure individuals attend community events.

Responsibilities

  • Provide written quarterly service plan reports on all assigned cases.
  • Participate in agency on-call rotation, including staffing coverage, transportation, and other duties.
  • Monthly contacts with individuals, including at least one contact in the community and in the home.
  • Weekly supervision/follow-up with Family Consultant Supervisor.
  • Supervise direct care staff and individuals.
  • Provide copies of Fire Drills and Supervisory check lists to Family Consultant Supervisor.
  • Serve as an advocate and teacher to the individuals.
  • Attend Monthly Unit Meetings and Monthly Staff meetings.
  • Rotate on the CRT (Critical Response Team) as needed.
  • Report incidents to CCS Individual Advocate/FC Supervisor within 24 hours.
  • Develop an understanding of the DBHDD system and Office of Regulatory Services rules and regulations.
  • Support the individual, their family, and supervision of direct care staff.
  • Aid in establishing valued social roles in the community.
  • Write SOAP notes and document them on Kaleidacare computer system.
  • Follow policy and procedures as outlined by Creative Community Services, Inc.
  • Ensure individuals attend social gatherings and provide transportation as needed.
  • Coordinate and/or provide transportation for individuals as needed.
  • Attend DD social events and CCS company events.
  • Complete 2 full days (9-5) in the office.
  • Meet deadlines for finance departments requirements, including timesheets, mileage, expense requests, leave requests, and Medicaid billing.
  • Ensure Medicaid summaries are accurate and complete by deadlines.
  • Filing is to be completed in a timely manner on a monthly basis or as documents are received.
  • Adhere to CCS Dress Code, wearing appropriate attire.
  • If coverage cannot be found for an individual, provide the coverage.
  • Train CA’s as needed.
  • Submit daily task list and weekly calendar to Supervisors.
  • Ensure completion of 40 hours of work per week, turning in leave requests if necessary.
  • Implement inclement weather protocol, contacting staff and families for safety measures.

Qualifications

  • An understanding and experience with foster care, developmental disabilities, and applied behavioral analysis.
  • A Bachelor’s Degree in a Human Services field or relevant experience.
  • Experience working with special needs children and/or persons with developmental disabilities.
  • Ability to analyze and react to difficult situations under stress and pressure.
