CASE MANAGER (CSS)

About the Role

Community-based position assisting individuals with service needs, developing strategies and supportive social support networks, and coordinating rehabilitation services as specified in the consumer's individual recovery plan.

Responsibilities

• Partner with the individual and CORE services provider in the development of the individual service plan.
• Assist the individual with transitioning between levels of care.
• Link and/or serve as an advocate with other agencies to garner needed resources for consumers.
• Provide assistance and support in crisis situations.
• Provide assessment, monitoring, and assistance to individuals.
• Establish professional and ethical relationships with individuals, families, and coworkers.
• Provide direct service to individuals; complete all necessary paperwork according to policy; ensure individuals meet eligibility criteria or refer to appropriate providers in the community.
• Manage a caseload by ensuring all services are authorized, chart paperwork is complete and current, and consumer charts are closed according to Policy & Procedure.
• See a caseload at the frequency defined in the service plan; document attempts to contact individuals and ISP reviews; complete monthly caseload reports; track appointments and case management.
• Other duties as assigned.
