Case Manager (CM), Social Worker (LSW)

About the role

Provides social service and discharge planning functions for the hospital. Offers resources for inpatients, outpatients, Emergency Department patients, and supports the Case Management team for psychosocial needs. Maintains performance improvement activities within the department.

Reports to: Director, Case Management | FLSA: Exempt

Requirements

• Bachelor's Degree in Social Work (Master's preferred)
• Current Social Worker license for the state of Mississippi
• Basic Life Support (BLS) certification within 60 days of hire
• Minimum 3 years' work experience in a similar field of social work

Responsibilities

• Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives
• Determines, prioritizes, provides, and/or arranges for needed internal and external services/interventions
• Participates in case reviews to evaluate case management and progress
• Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan
• Assists with discharge planning and processes
• Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan
• Collaborates with providers, nursing, and ancillary staff
• Engages in practice that complies with regulatory standards, patient safety, and performance improvement initiatives
