About the role
The Case Manager in the Emergency Department coordinates comprehensive, patient-centered care for individuals with acute and chronic health conditions. They facilitate interdisciplinary collaboration, ensure smooth transitions across care settings, and serve as an expert resource on care protocols, resource utilization, and regulatory standards. The role involves partnering with patients and families to identify healthcare issues, address barriers to self-care, and provide culturally sensitive education using health literacy principles. They utilize quality improvement methods to monitor interventions and ensure fiscally responsible care coordination across the continuum.
Qualifications
- Nursing degree (BSN preferred)
- At least 2 years of clinical nursing experience
- Knowledge of disease management, healthcare reimbursement, and levels of care
Skills
- Strong organizational, communication, and transitional planning abilities
- Proficiency in electronic systems
- Licensure as an RN and case management certification (preferred)
- BLS certification (required)
