Case Manager BSW

About the role

As a Case Manager BSW at Novant Health Huntersville Medical Center you will participate in the complex discharge planning process. Huntersville Medical Center is an award-winning 151-bed hospital in Huntersville, a family-friendly town in the Lake Norman area. This part-time position offers more flexibility in your schedule while still providing part-time benefits. The schedule rotates with three 8-hour shifts and one full weekend every six weeks. You must be able to orient Monday through Friday for four to eight weeks depending on experience. Novant Health offers leadership support to help grow your career in the healthcare field. Responsibilities

• Participate in the complex discharge planning process as a valued member of the healthcare team
• Collaborate with physicians and other members of the healthcare team to assess, plan, implement, and evaluate proactive, complex and safe discharge plans
• Collaborate with RN Case Managers for guidance with medically complex cases
• Serve as a resource for discharge planning and care coordination for psychosocially complex cases, working with MSW Case Managers as needed
• Responsible for facilitating coordination of care
• Maintain current knowledge of available community resources and post-acute care options
• Accept responsibility, authority and accountability for management of care in accordance with current policies and procedures, mission, vision, and values of Novant Health

Requirements

• Bachelor’s Degree in Social Work from a school accredited by the Council on Social Work Education
• Two or more years of professional social work experience or two or more years of healthcare experience, preferred
• Case Management or related experience, preferred

Skills

• Weekend and holiday coverage; must work one full weekend every six weeks
• Knowledge/competence of Microsoft Office products and keyboarding (preferred)
