Responsibilities
- Coordinate safe, timely, and clinically appropriate discharge planning for patients receiving inpatient psychiatric treatment.
- Work collaboratively with patients, families, treatment teams, community providers, and external agencies to facilitate continuity of care.
- Identify discharge needs and barriers, coordinate referrals and aftercare services, arrange transportation, and ensure all discharge planning activities are documented accurately and completed within established timeframes.
- Support successful patient transitions and positive treatment outcomes by proactively managing case coordination from admission through discharge.
Qualifications
- Education: Associate Degree required; Bachelor's degree in Social Work, Psychology, Human Services, or a related field preferred.
- Experience: One to two years of case management, discharge planning, behavioral health, or healthcare experience preferred.
- Demonstrate effective care coordination, referral management, documentation, and collaboration with multidisciplinary teams and community providers.
Physical Requirements
- May require walking, standing, sitting for prolonged periods of time.
- Occasionally lifts up to 10 pounds and carries small objects from waist to chest.
- Requires ability to work long periods on the phone; hear and speak clearly over the phone.
- Must be able to read and write and specifically record information and read medical records.
- Regularly needs to bend, stoop, and reach.
- Emotional stability necessary to perform the essential functions of the job.
- Requires vision and hearing sufficient to perform the essential functions of the job.
Skills

- Grammar and spelling proficiency.
- The ability to read and comprehend written instructions and follow verbal instructions.
- The ability to establish and maintain effective working relationships with employees, managers, referral sources, patients/families, community resources, etc.
- The ability to exercise professional independent judgment in planning own work.
- Flexibility with work hours to accommodate work volumes.
- Knowledge of utilization management and managed care environment is helpful.
