Case Manager, Broadway House Women's Shelter

About the role

The Case Manager at Broadway House Women’s Shelter will provide comprehensive services to stabilize the condition of homeless women, particularly those who are mentally ill and chemically addicted. They will conduct initial and periodic assessments, prepare psychosocial evaluations, obtain necessary documentation, and collaborate with clients to develop and achieve their goals. This role also involves coordinating with other social services staff, advocating for clients, and ensuring client stability post-goal attainment.

Responsibilities

• Conduct initial intake and periodic re-assessments of clients’ needs.
• Prepare and update psychosocial evaluations according to regulations.
• Obtain and maintain relevant documentation and release forms from clients.
• In collaboration with clients, revise independent living plans and set short-term and long-term goals.
• Aid clients in accessing community resources and referring them to appropriate services.
• Work with clients to overcome barriers to goal attainment.
• Support and train clients, coordinate with other social services staff, and recommend strategies to encourage full participation.
• Monitor clients' progress via scheduled contacts and sessions.
• Ensure updates to the CARES Database reflecting client requests for services.
• Act as a liaison and advocate for clients with external organizations related to education, healthcare, housing, legal issues, etc.
• Escort clients to appointments and assist with completing and processing benefit applications.
• Follow up with clients after achieving primary goals to ensure stability.

Requirements

• Bachelor's degree (B.A., B.S.W.) or Associate's degree with two years relevant experience, or four years of relevant experience.
• Bi-lingual in English and Spanish.

Pre and/or Post Employment Requirements

• Proof of negative results from TB test.

Pay

$47,174 - $51,500 annually

Schedule

Full-time (35 hours per week)
