Case Manager (Bright Changes Clubhouse)

Duties and Responsibilities

• Aid individuals with service needs, develop strategies, and build supportive social support networks.
• Coordinate rehabilitation services as outlined in the individual's recovery plan.
• Support individuals in crisis situations.
• Assess, monitor, and provide assistance to individuals.
• Establish professional and ethical relationships with individuals, families, and coworkers.
• Provide direct service to individuals, complete necessary paperwork according to policy, and ensure individuals meet eligibility criteria or refer to appropriate providers.
• Manage a caseload by ensuring all services are authorized, chart paperwork is complete and current, and consumer charts are closed according to Agency guidelines.
• Perform other duties as assigned.

Qualifications

• Bachelor’s degree in Psychology, Social Work, Counseling, or related field.
• Experience working with individuals with mental health issues and/or substance use disorders.
• Knowledge of community resources and services.
• Ability to establish and maintain positive relationships with clients, families, and colleagues.
• Strong communication and interpersonal skills.
• Proficiency in Microsoft Office Suite.
