About the Role

PHMC serves as both a direct service provider to individuals, families, and communities across the region and an intermediary agent—managing large-scale contracts, government and philanthropic partnerships, and multidisciplinary initiatives that require operational sophistication, strategic leadership, and deep mission alignment.

The Bridge Intensive Prevention Services (BIPS), an after-school program serving at-risk youth ages 10–19 and their families in West Philadelphia, seeks a Case Manager to provide person-centered, strengths-based, and trauma-informed support. Reporting to the BIPS Program Director, the Case Manager coordinates intake appointments, conducts home visits, obtains school records, connects families with services, and advocates for youth and families at court hearings. The role also facilitates life skills groups, supports on-site activities, and maintains administrative systems aligned with program and payer requirements.
