Responsibilities
- Clinical Interventions/Discharge Planning: Coordinate health care decisions and develop treatment plans to improve quality and outcomes.
- Care Planning Management: Manage care plans across the continuum to promote cost-effective care within the allotted time frame.
- Fiscal Management: Ensure payments of hospital-based services meet patient-related utilization management criteria.
- Payer/Referral Management: Implement safe and appropriate discharge plans and manage payer and referral relationships.
Qualifications
- Current Licensure per state guidelines in clinical or related discipline OR a Bachelor's or Master's in a health or human services discipline.
- Previous experience in Case Management and Discharge Planning preferred.
- CCM Certification preferred.
