Case Manager

Responsibilities

• Clinical Interventions/Discharge Planning: Coordinate health care decisions and develop treatment plans to improve quality and outcomes.
• Care Planning Management: Manage care plans across the continuum to promote cost-effective care within the allotted time frame.
• Fiscal Management: Ensure payments of hospital-based services meet patient-related utilization management criteria.
• Payer/Referral Management: Implement safe and appropriate discharge plans and manage payer and referral relationships.

Qualifications

• Current Licensure per state guidelines in clinical or related discipline OR a Bachelor's or Master's in a health or human services discipline.
• Previous experience in Case Management and Discharge Planning preferred.
• CCM Certification preferred.

Pay

• $75,000 - $112,000 (based on experience).
