About the role

Vivent Health is seeking a Case Manager to join our healthcare team. This role involves comprehensive and compassionate case management services to clients, collaborating with healthcare professionals to ensure the highest quality of care.

Responsibilities

  • Serve as a member of the internal care team, overseeing clients' medical, social, and emotional needs.
  • Conduct thorough assessments and develop individualized case management plans.
  • Collaborate with healthcare providers to coordinate and facilitate access to medical services.
  • Assist clients in accessing community resources, social services, support programs, and insurance coverage.
  • Provide timely and effective crisis intervention services.
  • Maintain accurate and up-to-date documentation of case management activities.
  • Work closely with interdisciplinary healthcare teams to achieve optimal health outcomes.
  • Provide health education and support to clients.
  • Assist in coverage and support other service lines as needed.

Requirements

  • Bachelor’s degree in Social Work, Psychology, or a related field.
  • Previous experience in case management preferred.
  • Experience working in healthcare and social service systems preferred.
  • Valid Government Issued Driver’s License required.
  • Familiarity with coordinating and managing patient care plans, ensuring comprehensive and integrated services.
  • Experience in providing effective crisis intervention services to individuals facing medical, social, or emotional challenges.
  • HIV experience in a not-for-profit environment preferred.
  • Ability to learn multiple databases and software systems.
  • Strong interpersonal and communication skills.
  • Empathy and understanding of diverse cultural perspectives.
  • Ability to work independently and collaboratively within a team.
  • Problem-solving and decision-making abilities.
  • Commitment to upholding ethical standards and confidentiality.

Pay

$51,000/annually - $58,000/annually (Depending on Experience)

Schedule

Works scheduled hours within the clinic setting as part of the internal care team.

Salary $51,000/annually - $58,000/annually (Depending on Experience) The final salary will be determined based on factors such as market data, internal equity, candidate experience and qualifications.
