Case Manager

About the role

Provide quality case management as part of a multidisciplinary treatment team. Serve as a communication liaison between family and the treatment team. Develop and implement discharge plans, ensuring housing, transportation, and aftercare arrangements. Participate in treatment planning meetings, communicating patient and family information, placement, and aftercare details.

Must be organized, able to work well with adolescent families, document proficiently, and demonstrate good phone etiquette.

Responsibilities

• Conduct thorough assessments of clients' needs and develop individualized discharge plans.
• Provide counseling and support in areas such as addiction and behavioral health.
• Utilize motivational interviewing techniques to empower clients in crisis management.
• Collaborate with healthcare providers and community organizations to ensure comprehensive care.
• Maintain accurate documentation and case records in compliance with regulatory standards.

Requirements

• Bachelor's Degree in Psychology or related field.
